Doctor's questions [ Anamnesis ]

Please, help me to put these questions right. I hope this topic will useful for other physicians because sometimes we meet patients who can communicate with us just English.

There are some typical questions ( my typical ones):

The first question is about complaints: could you tell me what are you complaints?
or what is wrong with you?
How long have you these problems?

What medicaments took you (because of these problems)?

Was the pain because of it better?

Came the pain suddenly or gradually?

How do you feel?

Do you have lost or gained your weight in the last time? Do you want it?

Have you appetite?

Do you sleep well?

Have you thirst?

Do you have stool regularly?

Do you have any pains by urination?

Do you have the cough? Как задать вопрос " Отделяется ли мокрота?" В медицинском словаре два слова sputum and phlegm, какое из них употребляемо?

Do you have allergy to medicaments?

Do you drink alcohol regularly? How much?

Do you smoke? How much?

What medicaments take you regularly?

Please tell me about your problems.
(Very often doctors ask: "What brought you here?" (Что вас сюда привело?)
Аленький Цветочек, I'd rather you not ask "What is wrong with you?" as this is an inflammatory phrase used when people are fighting as if to ask "Ты чё, больной?"

You can also say,
How long have you had this for?

Are you on any medication for this issue?

Did the medication help the pain?

Did the pain appear suddenly or gradually? OR Did the pain come suddenly or gradually?

Perfectly fine. You can also add "right now", or "overall"? (т.е. в целом)

Have you lost or gained any weight lately?

Ты имела в виду, хотели бы вы похудеть? Не совсем поняла, прости.

This works, but in 95% cases I will usually hear
-Is your appetite normal?
or
-How has your appetite been?

Fine.

Do you get thirsty?
OR, if you want to ask "right this minute", then - are you thirsty?

Fine.

Do you have any pain when urinating?

Do you have any cough?

Is your cough productive? OR Do you produce any phlegm when coughing?
(both words, phlegm and sputum, are in use)

Are you allergic to any medicines?

Fine as it is.

You can add, how many cigarettes a day?

What medicines/medications do you take regularly, if any?
Are you on any medication?

Have you lost or gained any weight lately?

Ты имела в виду, хотели бы вы похудеть? Не совсем поняла, прости.

I mean, I should know if the Patient wanted to loose his weigh or not. Sometimes people want to became slimer and loose their weigh consciously. But sometimes people loose their weigh because of a diseases. They had any intend to loose their weigh.

OK, in this case what you should ask is,

Was your weight loss planned?
or
Were you trying to lose weight?

The very first question could also be "What seems to be the problem?"

I would like to share with you some questions. I found these questions in the small “English pockettool” which was presented me from one colleague.

What is the name of your primary care physician?
Do you have your insurance card with you?
What caused the wound you have?
Did you have a car accident?
Did you sustain cuts- impact/injuries/crush injuries?
Did you have a fall?
Have you been beaten? (stick/stone/fist)
Have you been stabbed?
Have you broken a bone?
Have you been shot?
Have you been bitten by a dog?
Did an insect sting you?
When did this occur?
How long have you been sick?
Are you vaccinated against tetanus?
Your last vaccination?
Do you feel depressed/ weak /tired?
Do you have any discomfort? In which area? Did you receive treatment for your ailment? Was it successful?
Have you been previously hospitalized? When? Why?
Have you ever experienced drug intolerance? Are you allergic to something? (medications/penicillin/ food products/ pollen /animals/latex band-aids/ insect stings/iodine/x-ray contrast agents).
Do you have headaches? Constantly/ occasionally?
Did you loose consciousness?
Did you vomit?
Do you have an eye problem? Which problem in particular?
Do you have impaired vision (temporary)( loss of eyesight/ double images/ blurred vision/ watery eyes/ pain /inflammation/ sensitivity to light/ black spots before eyes / light flashes)?
